Home Care Packages

Home Care Packages (HCPs) provide care and support services such as:

  • Nursing
  • Domestic assistance
  • In-home respite
  • Personal care
  • Transport to appointments
  • Social support

The type of support and health care we need at home is as individual as we are, and changes with our circumstances.

A Home Care Package (HCP) provides services to help you remain living at home for as long as possible. Packages offer choice and flexibility for recipients and are tailored to meet a range of care needs.

HCPs are subsidised by the Australian Government and cover higher level care and support services.

There are four levels of HCPs available, depending on the amount of support required:

  • Level 1: 

    Basic care package

  • Level 2:

    Low level care package

  • Level 3:

    Intermediate level care package

  • Level 4:

    High level care package

At Home Nursing Group we work closely with you to understand what your exact needs are, and we understand that those needs might change over time. Our expert staff can take care of your budget, funding and invoicing on your behalf, so you can focus on the things that matter to you.

Home Nursing Group has low administration fees and is able to maximise the funding available. We help you get more care hours from your funding.

All of our packages are offered on a Consumer Directed Care (CDC) basis. CDC offers more choice and flexibility for you to decide what services you wish to receive and when. Out team will talk with you about creating the independent and supported life you want. You’ll get the most from your funding, and we’ll take care of the details.

 

Are you eligible?

  • 65 years or older, or an Aboriginal or Torres Strait Islander person over 50 years of age.
  • Need support to live at home.

To receive a package, you must first be assessed by the Aged Care Assessment Team (ACAT). Home Nursing Group can assist you with this process. Once you have been successfully allocated a package, we walk you through a range of care options and help set goals that feel right for you.

Because HCPs belong to the care recipient rather than the care provider, you can compare providers to find one that best meets your needs.
