Activities Facilitator

Home Nursing Group is a locally owned and operated Commonwealth Approved Provider of community Aged Care services, supporting older people to live safe, healthy and happy lives in their homes.

We are currently recruiting for an Activities Facilitator to assist in the day-to-day running of the Jacaranda Social Club program. Our ideal candidate will have initiative, people skills and a desire to deliver exceptional customer service.

Following recent significant growth in the business, this position will provide consistent and effective day to day facilitation of the Jacaranda Social Club program under direct guidance from the Care Manager New England. They would maintain client and stakeholder engagement with this social support function of the business, and provide ‘hands on’ management of the day to day program.

QUALIFICATIONS AND SELECTION CRITERIA

  1. Relevant health related qualifications and experience applicable to the aged care sector, such as Certificate III in Aged Care and/or Certificate III in Home and Community Care or equivalent
  2. Practical experience within the health care sector dealing in a compassionate and efficient manner with clients and their families
  3. A demonstrated high level of communication and interpersonal skills including both oral and written communication including proficiency with documentation pertaining to client care.
  4. Demonstrated communication skills specific to case management such as case conferencing, interviewing and assessment techniques
  5. Demonstrated time-management skills and the ability to apply problem-solving skills.
  6. Willingness to participate in the ongoing development of wellness approaches and consumer directed care.
  7. Demonstrated experience with a range of computer software packages including the suite of Microsoft programs.
  8. Demonstrated financial acumen.
  9. Demonstrated customer service skills.
  10. Demonstrated ability to successfully undertake duties and achieve agreed objectives within a team environment as well as independently applying own initiative.
  11. Demonstrated knowledge and understanding of EEO, WHS, risk management, quality assurance and records management requirements as they relate to this position.
  12. Demonstrated understanding of cultural sensitivity
  13. Hold a current NSW driver license.
  14. Current National Police Check
