Position Description

  • Reports directly to the New England Clinical Programs Regional Manager
  • Location – Armidale
  • Pay rate – as per award
  • Hours – permanent part time
  • Probationary period – 6 months

This position shall work autonomously in assessing, delivering and monitoring high quality nursing care and case management to clients with supervision by the Regional Manager in accordance with accepted nursing practice and standards. The position will participate in services with Home Nursing Group team to ensure equitable service delivery of home nursing services.

The position reports directly to the New England Clinical Programs Regional Manager.

Position Responsibilities

The incumbent will work under the broad direction of the New England Clinical Programs Regional Manager, however specific responsibilities will include;

  1. Adhere to the policies, principles and procedures of HNG in accordance with stated organisational policy.
  2. Provide a range of flexible services to clients living in their own homes based on individual assessment, established procedures and best practice and assist in evaluating care plans to meet changing needs as required.
  3. Utilise a broad range of nursing skills and knowledge to identify patient and Carer needs and problems which may require specialised care plans.
  4. Maintain a high standard of nursing documentation which accurately reflects professional standards of care delivery associated with Home Nursing Group.
  5. Interact and collaborate effectively with all members of the care team to
  6. contribute to the delivery of quality care for all clients.
  7. Respond to altered health status in clients by providing clinical guidance and realigning clients care and services as needed.
  8. Update relevant skills and undertake formal opportunities for training and skills improvement as relevant to Home Nursing Group.
  9. Development and maintenance of relationships with key stakeholders including but not limited to; the Department of Social Services, other community care organisations, general practitioners, clients and their carers.
  10. Meeting statutory reporting requirements as per Home Care Standards and DSS guidelines.
  11. Provide clinical expertise as a member of the clinical on-call team for after hour’s advice on a roster basis.

Qualifications and selection criteria


  1. Registered Nurse currently registered with the Australian Health Practitioner Regulation Agency (AHPRA). A minimum of two years post-graduate experience, preferably in the area of Aged Care and community care setting.
  2. Ability to delegate and prioritize tasks.
  3. Have a sound working knowledge of continuous quality improvement, risk management, EEO principles and WHS as they relate to the position.
  4. Current CPR certificate.
  5. Current unencumbered NSW Driver’s License and own registered vehicle.
  6. Current National Police Check (to be undertaken by HNG).
  7. A strong commitment to working with frail aged people and people with disabilities.
  8. Ability to work independently and without direct supervision.
  9. Demonstrated organisational and time management skills.
  10. Experience in a service coordination role.


  1. Experience working in a clinical community setting.
  2. Ability for occasional weekend and after-hours work, as required.
