Employment Criteria – Customer Relations

The criteria listed below are requirements for Home Nursing Group Customer Relations Staff. Please address each criteria as part of your application.

  1. Relevant health related qualifications and experience applicable to the aged care sector, such as Certificate III in Aged Care and/or Certificate III in Home and Community Care or equivalent
  2. Relevant Case Management and care coordination skills/ certification including advocacy, care planning, goal setting and experience with networking and collaboration around patient care.
  3. Demonstrated oral and written communication skills and communication skills specific to case management such as case conferencing, interviewing and assessment techniques.
  4. Demonstrated time-management and delegation skills and the ability to apply problem-solving skills.
  5. Demonstrated ability to  participate in the ongoing development of wellness approaches and consumer directed care.
  6. Demonstrated experience with a range of computer software packages including the suite of Microsoft programs.
  7. Demonstrated financial acumen.
  8. Demonstrated ability to successfully undertake duties and achieve agreed objectives within a team environment as well as independently applying own initiative.
  9. Demonstrated customer service skills
  10. Have a sound working knowledge of continuous quality improvement, risk management, EEO principles and WHS as they relate to the position.
  11. Current NSW Driver’s License and own registered vehicle.
  12. Current National Police Check
  13. Availability for after-hours support on an on-call roster basis.
