Care & Support Coordinator Armidale Selection Criteria

Qualification and selection criteria

1. Relevant health related qualifications and experience applicable to the aged care sector, such as Certificate III in Aged Care and/or Certificate III in Home and Community Care or equivalent
2. Practical experience within the health care sector dealing in a compassionate and efficient manner with clients and their families
3. Certificate IV in Case Management or willingness to undertake
4. Relevant Case Management and care coordination skills including advocacy, care planning, goal setting and experience with networking and collaboration around patient care.
5. Demonstrated oral and written communication skills and communication skills specific to case management such as case conferencing, interviewing and assessment techniques.
6. Ability to prioritise and delegate tasks.
7. Demonstrated time-management skills and the ability to apply problem-solving skills.
8. Willingness to participate in the ongoing development of wellness approaches and consumer directed care.
9. Demonstrated experience with a range of computer software packages including the suite of Microsoft programs.
10. Demonstrated financial acumen.
11. Demonstrated ability to successfully undertake duties and achieve agreed objectives within a team environment as well as independently applying own initiative.
12. Demonstrated understanding of cultural sensitivity.
13. Have a sound working knowledge of continuous quality improvement, risk management, EEO principles and WHS as they relate to the position.
14. Current unencumbered NSW Driver’s License and own registered vehicle.
15. Current National Police Check
16. Availability for after-hours support on an on-call roster basis.
