Care and Support Coordinator

POSITION DESCRIPTION

This position is designed to support and coordinate HACC and other low care services within the Mid North Coast office of Home Nursing Group.  The position reports directly to the Regional Manager, Mid North Coast.

    Suite A, Level 1
    144-148 West High Street, Coffs Harbour
  • Pay Rate: $25.00 – $28.00 per hour (dependent on skills and experience)
  • Hours: Part-time  (approx. 24 hours per week) – 12 month contract position
  • Probationary period: 6 months

POSITION RESPONSIBILITES

The position provides “hands on” support within the Mid North Coast regional office so as to help ensure that all Home and Community Care (HACC) services are coordinated and delivered in a timely and professional manner. The incumbent will work under the broad direction of the Regional Manager, Mid North Coast, however specific responsibilities will include:-

  1. Assessment and support coordination of HACC clients in the Mid North Coast region
  2. On-going management of HACC clients, including assessments, planning and reviews
  3. Ensure accurate records are entered and maintained in the client data system (currently “Rosterit”, with “Procura” to be implemented by end of 2014)
  4. Ensure all HACC hours and expenditure are kept up to date and recorded
  5. Develop and maintain relationships with clients and their carers, community care groups and other stakeholders as appropriate including promotion of Home Nursing Group’s HACC services
  6. Providing direction and guidance to AINs and domestic assistants where appropriate
  7. Undertake “on-call” duties as required
  8. Participate in quality assurance activities within the service delivery sphere and general office environment
  9. Attend all mandatory training and participate in staff performance reviews
  10. Be aware of and apply Home Nursing Groups Policy and Procedures relating to Quality Assurance, Risk Management, Work Health Safety, Records Management and all other relevant policies and as they apply to this position.
  11. Undertake other duties as required and relevant to the position.

QUALIFICATIONS AND SELECTION CRITERIA

  1. A demonstrated high level of communication and interpersonal skills including both written and oral communication
  2. Demonstrated  time management and organisational skills with an ability to apply problem solving skills
  3. Practical experience within the aged care sector dealing in a compassionate and efficient manner with clients and their families
  4. Relevant health related qualifications and experience applicable to the aged care sector, such as Certificate III in Aged Care and/or Certificate III in Home and Community Care or equivalent
  5. Relevant experience with care coordination or similar
  6. Diploma in Community Service Coordination or willingness to undertake
  7. Demonstrated experience with a range of computer software packages including the suite of Microsoft Programs
  8. Demonstrated ability to successfully undertake duties and achieve agreed objectives within a team environment as well as independently applying own initiative
  9. Demonstrated knowledge and understanding of EEO, WHS, risk management, quality assurance and records management requirements as they relate to this position
  10. Hold a NSW driver license
