Care and Support Coordinator – Port Macquarie

Position Description

  • Reports to the Mid North Coast Regional Manager
  • Location – Port Macquarie
  • Pay Rate $27.00 – $30.00 per hour (dependant on skills and experience)
  • Hours – Permanent full-time or part-time position
  • Probation period 6 months

This position is designed to support and coordinate HACC and other care services within the Mid North Coast region, specifically Port Macquarie and surrounding areas.  The position reports directly to the Mid North Coast Regional Manager.

Position Responsibilities

The position provides “hands on” support within the Mid North Coast region of home Nursing Group (HNG) so as to help ensure that all Home and Community Care (HACC) and other services are coordinated and delivered in a timely and professional manner. The incumbent will work under the broad direction of the Mid North Coast Regional Manager, however specific responsibilities will include:

  1. Assessment and support coordination of HNG clients in the Mid North Coast region
  2. On-going management of HNG clients, including assessments, planning and reviews
  3. Ensure accurate records are entered and maintained in the client data system (currently “Rosterit”, with “Procura” to be implemented by mid of 2015)
  4. Ensure all service hours and expenditure are kept up to date and recorded
  5. Develop and maintain relationships with clients and their carers, community care groups and other stakeholders as appropriate including promotion of Home Nursing Group’s HACC and other services
  6. Providing direction and guidance to AINs and domestic assistants where appropriate
  7. Provide administration support where appropriate to the role including filing, printing, petty cash and banking client contributions.
  8. Provide support as a member of the on-call team for after hour’s advice on a roster basis.
  9. Participate in quality assurance activities within the service delivery sphere and general office environment
  10. Attend all mandatory training and participate in staff performance reviews
  11. Be aware of and apply Home Nursing Groups Policy and Procedures relating to Quality Assurance, Risk Management, Work Health Safety, Records Management and all other relevant policies and as they apply to this position.
  12. Undertake other duties as required and relevant to the position.

Qualifications and Selection Criteria

  1. A demonstrated high level of communication and interpersonal skills including both written and oral communication
  2. Demonstrated  time management and organisational skills with an ability to apply problem solving skills
  3. Practical experience within the aged care sector dealing in a compassionate and efficient manner with clients and their families
  4. Relevant qualifications and experience applicable to the aged care sector.
  5. Relevant experience with care coordination or similar
  6. Diploma in Community Service Coordination or willingness to undertake
  7. Demonstrated experience with a range of computer software packages including the suite of Microsoft Programs
  8. Demonstrated ability to successfully undertake duties and achieve agreed objectives within a team environment as well as independently applying own initiative
  9. Demonstrated knowledge and understanding of EEO, WHS, risk management, quality assurance and records management requirements as they relate to this position
  10. Hold a NSW driver license
