Home Care Packages (HCPs) are subsidised by the Australian Government and are available for people over 65 (over 50 for people from Aboriginal and Torres Strait Islander communities) who need support to live at home.
The type of support and health care we need at home is as individual as we are, and changes with our circumstances. Therefore, all Home Nursing Group HCPs are delivered on a “Consumer-Directed-Care” (CDC) basis. Compared with traditional home care packages, CDC offers more choice and greater flexibility for you to decide what services you wish to receive and when. HCPs can provide both low level support (Level 1 and 2 packages) to high level complex care needs (Levels 3 and 4).
After you have been assessed by your Aged Care Assessment Team (ACAT) as eligible for an HCP, you are ready to choose your care. Home Nursing Group will assist you with this process, and we will walk you through a range of care options and set goals that feel right for you.
By asking questions about your quality of life, we seek to ensure that the support we offer best suits your needs, bringing you better health and well-being. For example:
HCPs cover care and support services such as nursing, domestic assistance, in-home respite, personal care, transport to appointments, and social support. All packages can also be tailored for people living in the community who may experience behaviours and symptoms associated with dementia that affect their ability to live independently. We will work with you to develop a tailored package of services to meet your needs and goals; as well as a budget showing the cost of services. We will monitor and review these with you on a regular basis.
The Australian Government provides a daily subsidy per package to supply and coordinate care services. In addition, you will be asked to contribute towards the cost of your care. The amount of your contribution is determined by your financial status and is modest.
